Immunization Reporting Help
For 2019-20 school year




This session will be with all participants sitting at a computer and learning how to complete the state immunization report. The participant will complete and submit their report.

Please have your login information.


Important Session Information:


This class will assist you to fill out and submit your annual immunization report while you are here. You must bring all of your immunization information with you to class, either printed out or it can be retrieved from a computer. You may bring your own laptop, but we will be in a computer classroom so it isn't necessary.
