Empowering Transition Success: Assessments, Coordinated Set of Activities, and Postsecondary Goals




This comprehensive training will empower educators to support students with disabilities as they transition to life beyond school. We will explore three critical components of the transition planning process: Transition Assessment, Coordinated Set of Activities, and Writing Appropriate Measurable Post-Secondary Goals. Throughout the session, participants will gain the knowledge and practical skills to conduct effective transition assessments, develop coordinated sets of activities, and write meaningful, measurable post-secondary goals. By the end of this training, you will be better equipped to foster student involvement and the essential support needed for a seamless transition from school to post-school life.
