Transition 101/ Top Tips




An in-depth look at the Federal and Texas Transition process used in their child’s IEP to help them be successful after graduating from high school. This training will cover the importance of self-determination and involving your child in the transition process. This session will also cover an overview of the transition process to include:

  • Transition assessments;
  • Annual individual education program (IEP) goals and post-secondary goals;
  • Present levels of academic achievement and functional performance (PLAAFPs);
  • Course of study;
  • Coordinated set of activities;
  • Self-determination;
  • Summary of Performance; and
  • Agency Linkages.
