Transition in Practice: Building Coordinated Activities




The "Coordinated Set of Activities" is often the most misunderstood section of the transition plan. This session breaks down how to move past generic "boiler-plate" statements to create a robust, individualized roadmap. We will explore how to connect classroom instruction to real-world community experiences and agency linkages that truly prepare students for life after high school.

Important Session Information:


Virtual Format: This is a 100% online session. A stable internet connection and a device with a camera/microphone are recommended for full participation.

Meeting Access: The Zoom link will be sent to the primary email address listed in your ESCWorks profile the afternoon before the session and again the morning of the meeting.

Action Required: Please log in to ESCWorks before the session to verify that your primary email address is current.

Materials: Digital handouts will be provided prior to the session and shared in the chat.

Coaching: Follow-up coaching is available (in-person or virtual, individual or small group) at your convenience to support implementation on your campus.
