Empowering Transition Success: Assessments, Coordinated Set of Activities, and Postsecondary Goals




This comprehensive training will empower educators to support students with disabilities as they transition to life beyond school. We will explore three critical components of the transition planning process: Transition Assessment, Coordinated Set of Activities, and Writing Appropriate Measurable Post-Secondary Goals. Throughout the session, participants will gain the knowledge and practical skills to conduct effective transition assessments, develop coordinated sets of activities, and write meaningful, measurable post-secondary goals. By the end of this training, you will be better equipped to foster student involvement and the essential support needed for a seamless transition from school to post-school life. Important Session Information:


Session Overview: Join us for an interactive, full-day workshop designed to streamline the transition process. You will examine practical examples, collaborate with peers, and get dedicated time for hands-on application and personalized support.

What to Bring: Please bring a laptop/device and current transition data, IEP drafts, or transition plans for 1–2 students on your caseload. Having quick access to these materials will allow you to practice, refine, and apply strategies directly to your work during the session.

On-Site Support & Follow-Up: Beyond dedicated work time with real-time feedback during the session, attendees are eligible for customized follow-up consultation, available face-to-face or virtually, individually or in small groups, to support ongoing implementation.

Location & Materials: This is an in-person session. Digital and printed resources will be provided on-site.
